List the product names and quantities of each item that you would like (please print). You can call us at the telephone numbers listed under Direct Telephone Orders to find out the exact cost including shipping (plus sales tax for Washington residents and sales tax for nutritional products for California residents).

Enclose a check or a charge card number (including the card expiration date) and include the following information:

• Ship-to Name
• Ship-to Address
• Ship-to City, State, and Zip Code (or equivalent for destinations outside the U.S.)
• Country and Country Code (for international orders)
• Phone Number
• E-mail Address (for on-line order confirmation and to receive occasional health bulletins)
• Visa or Master Card Number and Expiration Date (if using a charge card)
• Name on Visa or Master Card (if using a charge card)
